Attaching Surveys to Actions, Events, or Locations
Use surveys to collect information from your users, with a variety of question types, export capabilities, and reporting functionality this is perfect for a variety of feedback and submissions your organization might want to ask users.
Here are a few ideas for Surveys:

Visibility from the Home Screen: Users can see all enabled surveys which have a release date of the current date or prior.
Visibility from a Category: Users can see all enabled surveys but only surveys with a release date of the current date or prior will be accessible.
HIPAA Compliance: Surveys can not be used as a part of a HIPAA compliant solution. We recommend creating an action linking to an external compliant service.
When creating a new survey you are required to add a name, the first date a user will be allowed to take the survey, and then the points a user should earn for completing the survey. Watch the video below to better understand how to add in questions.
Cover Image: When adding a Cover Image, the recommended size is a ratio of 16:9 landscape. If you do not add your own image, then your category's image will appear instead.
If an action you created includes a Survey that should be completed after the Action, users may have a hard time spotting the survey. We recommend setting your 'Call to Action' to something like 'Take This Survey' to let users know they will be directed to another page.
Once the Call to Action Button is pressed, the user will earn points for the action and a new command will ask users "Would You like to Take Our Survey _" with the survey name you attached. The option to open the survey will replace the Call to Action button and will be available to click for a few moments.
If Users were unable to press 'Open' the first time, No worries! They always have the option to press the 'I didn't Do This' button that will appear in the same spot as the Call to Action button.
Quickly see a list of users who have recently responded and export all of their responses to evaluate externally.
When you export the responses you will receive a CSV file that includes the following fields
Survey Name
User ID
User Email
User Name
The name of the question
The answer given
The answer date
If this data is not sufficient or if you would like further data analysis power please contact your customer support specialist who will walk you through more data plans.
View a summary of user responses by visiting the "Results" tab. You might need to hit the "Show Results" button on a user’s response to view all the data on the reports tab.
Survey Not Visible in App: Check to see if the survey has is an assigned segment group. Users who are not in specified segments will not be able to view the action or activity.
Survey Not Loading in App: A common issue is a survey not loading when you try to take the survey in the app. This happens when you set the survey date to a future date.
CSV Export is Blank: Our system is running a little slow until the migration is completely finished. Before a csv can be exported our system needs to load the other components of the Admin like users, categories etc. Just wait a minute and it should work fine!
Survey Questions Not Saving: Right now our survey system doesn't allow for question modification. If you would like to alter a survey question we recommend deleting the question from the survey and adding a new question.
